Congestion levy forms move online

The following paper-based congestion levy forms have been replaced with online SmartForms:

• Exemption claim form (CGL-Form-01)
• Change of details (CGL-Form-02)
• Concession claim form (CGL-Form-05)
• Application for registration as an owner or operator of a car park for Congestion Levy (CGL-Form-06)

In addition:

• The Application for registration of an operator for a public car park (CGL-Form-03) and Application for registration of an owner (CGL-Form-04) forms have been removed from our website and incorporated into CGL-Form-06.
• The Annual Return 2017 form (CGL-Form-00) has been removed from our website (please contact us if you require this form)

Old PDF versions of these forms will not be accepted beyond 30 September 2017.
