Updated customer form

The SRO has released an updated customer form which has been amended due to changes introduced by the Building a Better Victoria (State Tax and other Legislation Amendment) Act 2014.

Please note the following changes:

Duties-Form-40 Insurance Duty – Application for registration as an insurer replaces SRO Duties Form 40

• This form must be used by insurers to register under section 186 of the Duties Act 2000.
