Lodge an FOI access request
Do I need to lodge an FOI request?
Often there are other ways to access documents without the complexities associated with an FOI request.
For instance, we already publish much information on this website and many queries can be addressed by phoning us on 13 21 61.
If you are considering lodging an FOI request, please contact our Information Officer Sue Kaufmann by phone on 03 9628 0228, fax on 03 9628 0576, or by email at email@example.com.
How do I lodge an FOI request?
Requesting access to documents under the Victorian Freedom of Information Act 1982 (the Act) must be in writing and addressed to:
State Revenue Office Victoria
GPO Box 1641
Melbourne VIC 3001
Make sure you clearly describe the documents to which you seek access and include payment of the statutory application fee or evidence in support of waiving the fee (for example, a valid Pensioner Concession Card or Health Care Card). Cheques should be made payable to the State Revenue Office. Payments can also be made online. Please contact the Information Officer for details.
There is no prescribed application form. If you are making a request on behalf of another party, please include a signed authority from them to show us you have their permission to obtain the requested information.
From 1 July 2018 to 30 June 2019, the statutory application fee for lodging an FOI request is $28.90.
When we receive a valid request, we are required to provide an access decision as soon as practical, but no later than 30 days after receiving the request. In some cases, this time may be extended. Additional access charges may apply.
