How do I lodge an FOI request?

Requesting access to documents under the Victorian Freedom of Information Act 1982 (the Act) must be in writing and addressed to:

FOI Requests
State Revenue Office Victoria
GPO Box 1641
Melbourne VIC 3001

Make sure you clearly describe the documents to which you seek access and include payment of the statutory application fee or evidence in support of waiving the fee (for example, a valid Pensioner Concession Card or Health Care Card). Cheques should be made payable to the State Revenue Office. Payments can also be made online. Please contact the Information Officer for details.

There is no prescribed application form. If you are making a request on behalf of another party, please include a signed authority from them to show us you have their permission to obtain the requested information.

From 1 July 2019 to 30 June 2020, the statutory application fee for lodging an FOI request is $29.60.

When we receive a valid request, we are required to provide an access decision as soon as practical, but no later than 30 days after receiving the request. In some cases, this time may be extended. Additional access charges may apply.
