How do I lodge an FOI request?
Requesting access to documents under the Victorian Freedom of Information Act 1982 (the Act) must be in writing.
During Victoria's State of Emergency, we prefer to receive requests via email at firstname.lastname@example.org. However, you can also submit a request via post:

You do not need to complete an application form, but make sure you clearly describe the documents you are requesting and include your contact details.
If you are making a request on behalf of another party, please include a signed authority from them to show us you have their permission to obtain the requested information.
When making a request, include proof of payment for the statutory application fee or evidence in support of waiving the fee, such as a valid Pensioner Concession Card or Health Care Card.
From 1 July 2020 to 30 June 2021, the statutory application fee for lodging an FOI request is $29.60. Payment can be made by electronic funds transfer (EFT). Please contact our FOI team for payment details.
We aim to provide an access decision within 30 days of receiving a valid request, although some decisions may take longer. We will notify you if we are unable to provide a decision within 30 days.
