About this form

Use this form to claim a congestion levy exemption for a parking space.

This form must be completed by the occupant of the car park. A separate form is needed for each premises.

This form is not automatically submitted to us. After completing the form, you must print and sign it, then send it to the car park owner or operator for their records. The car park owner or operator uses the information to support an exemption claim.

Get these details ready

You will need:

  • the address of the car park with the exempt spaces
  • the type and number of exemptions being claimed
  • business details if applicable, including your Australian Business Number, Australian Company Number or State Revenue Office customer number
  • total number of spaces leased or occupied
  • occupant contact details.

For parking for shift workers, you also need details about the 24-hour business and the employees who may use the exempt spaces.
