• an authorised representative of a trust that owns or rents the premises.

• the details of the property owner or renter (individual, company/incorporated association or trust), including their postal address and contact details
• property address
• basis for why the property is not short stay accommodation
• a separate schedule if you are providing a declaration for multiple properties.

You must give the completed declaration to any booking platforms that accept short stay bookings on your behalf. Do not send the form to us.

The booking platform must keep this declaration for 5 years.
