Use this form to nominate a member of a group of companies as the designated group employer for payroll tax. Submitting this form will replace any previous nomination.

The designated group employer or their authorised representative can submit the form. 

You can save the form and return to it later by using your tracking code and answering your security question.

Get these details ready

You will need:

• details of the group member being nominated as the designated group employer
• details of other members of the payroll tax group
• Australian Business Numbers (ABNs), Australian Company Numbers (ACNs) and State Revenue Office customer numbers
• the relevant group dates, including the effective date of the nomination.
