Use this form to tell us when administration of a deceased estate starts and ends.
When administration starts, you must lodge the form within a month of the legal personal representative being appointed
When administration ends, you must lodge the form within a month of the administration being completed.
Review the process for:
- commencement of administration of a deceased estate
- completion of administration of a deceased estate.
You can complete this form if you are:
- the personal representative of a deceased estate
- the authorised representative acting on behalf of a personal representative.
If there is more than one personal representative, only one person needs to complete this form.

Get these details ready
When you start administration, you will need:
- your personal details, including your address, date of birth and State Revenue Office customer number (if known). If there is more than one personal representative, you will need details for each.
- the deceased’s details, including residential address at their time of death, date of death and State Revenue Office customer number (if known).
When you complete administration, you will need:
- your name and address
- the name of the deceased estate, date of death and State Revenue Office customer number (if known)
- the date administration was completed
- the details of all land held by the deceased estate at completion of administration.
