About this form
This form can be used to nominate someone to interact with us on your behalf. This can be anyone, including a family member, your accountant or solicitor.
They will be able to access information and correspondence about all the taxes or levies you authorise them for. If you authorise a representative for land tax, their access will include all land you own. This includes your principal place of residence and any other land, such as land you own jointly or through a trust.
We use the information provided to update our records and may, where authorised, share the changes with other government agencies. Your changes will stay in effect until you advise us otherwise.
This form can be completed by either:
- a State Revenue Office customer
- an agent or representative if they have previously been authorised to act on the customer's behalf.
Customers updating their own details should use the Change of contact details (GEN-Form-03) form.
Car park owners or operators (or an authorised representative) registered for the congestion levy should use the congestion levy (CGL-Form-02) to change owner or operator details.
Get these details ready
You may need to enter the following:
- the SRO customer number, if available
- the full name and contact details of the representative you are adding or updating
- the organisation name, if the representative is acting for a business
- the tax or levy the authorisation applies to
- a signed and dated authority from the customer, if it has not already been provided.
