Change of contact details

About this form

Use this form to update your contact details if you cannot update them in your online account. 

If you have an online account for your tax or levy, you should update your contact details in that portal instead. 

• For land tax, use My Land Tax. 
• For vacant residential land tax, use the vacant residential land tax portal. 

Any changes to your records as a result of submitting this form will stay in effect until you advise us otherwise.

This form can be completed by a State Revenue Office customer or their agent or representative if they have previously been authorised to act on the customer's behalf. For new authorisations, use the authorise or update a representative (GEN-Form-09) form. 

Car park owners or operators (or an authorised representative) registered for the congestion levy should use the congestion levy (CGL-Form-02) to change owner or operator details. 

Get these details ready

You may need to enter the following: 

• your customer number, if you have one 
• your ABN or ACN, if completing the form for a company or trust 
• the tax or levy the change applies to 
• the new contact details you want us to update. 

If you are acting on behalf of a customer and are not already authorised as their representative, you will need a signed and dated authority from the customer.
